Complaints Submission Form
Use this form to submit a formal complaint (Stage 2) under the Complaints Policy (DAIS-POL-003).
You can complain about the quality of teaching or learning materials, the conduct of staff,
an assessment decision, an administrative process, a technical failure that significantly
affected your learning, or any other matter relating to quality of provision. Before submitting
a formal complaint, you are encouraged to first raise your concern informally with your tutor
or the Complaints Officer (Stage 1). You will not be disadvantaged for complaining in good faith.
